I, the undersigned parent or legal guardian of the camper named above, hereby give permission for my child to

participate in the STEM Summer Camp organized by IHUB STEAM Academy ("the Camp"). I understand that

STEM camp activities may include hands-on science experiments, basic engineering projects, supervised

technology use, and interactive learning sessions.

I acknowledge and accept that participation involves certain risks, including but not limited to minor cuts, burns,

allergic reactions, or physical activity-related injuries. I voluntarily assume all such risks and agree to release and

hold harmless the Camp, its staff, volunteers, and associated organizations from any and all liability, claims, or

demands arising out of or relating to my child’s participation.

Medical Consent

In the case of a medical emergency, I authorize the Camp staff to seek appropriate medical care for my child if I

cannot be reached in time. I understand that the Camp will make every effort to contact me before taking such

action.

Technology and Equipment Use

I understand that my child will use various STEM-related tools, software, and equipment. I acknowledge that all

equipment will be used under adult supervision and agree to instruct my child to follow all safety guidelines

provided by the Camp staff.

All students will be required to bring their own laptops to participate in computer-based classes. Please ensure

the device is fully charged daily and suitable for basic STEM-related tasks such as coding, simulations, and

presentations.

Photo/Media Release

For the safety of all campers, only the individuals listed below will be authorized to pick up or drop off my child. A

valid photo ID will be required at the time of pick-up. If someone other than the listed individuals needs to pick up

my child, I will notify the Camp staff in writing in advance.

Code of Conduct

Welcome to STEM Summer Camp! We’re here to explore, create, and have fun together! To keep everyone safe and

happy, all campers must follow these important rules.

Be Respectful

Be kind with your words and actions.
Listen when others are talking.
Treat everyone the way you want to be treated.
Respect other people’s space and belongings.

Be Safe

Follow all safety rules, especially in the lab or when using tools.
Walk indoors - no running or horseplay.
Tell an adult right away if someone gets hurt or something seems wrong.

Try Your Best

Join in all activities with a good attitude.
Ask questions, help others, and don’t be afraid to make mistakes - that’s how we learn!
Be proud of your work and try to do it on your own.

Use Technology the Right Way

Use computers, tablets, and other tools only when allowed.
Don’t change settings or visit websites unless a teacher says it’s okay.
Keep screens out of free time unless given permission.

Take Care of Camp Spaces

Clean up after yourself.
Take care of tools, materials, and equipment.
Leave things better than you found them!

What’s Not Okay

No bullying, teasing, or leaving others out.
No hitting, pushing, or hurting anyone — ever.
No stealing, damaging, or messing with other people’s things.
No bad language or rude gestures.
No cheating or copying someone else’s work.

If Rules Are Broken

Camp staff will:

Give a reminder or warning.
Talk to you and help you make better choices.
Call your parents or guardians if needed.
Ask you to leave camp if the problem is serious or doesn’t stop.
